Voice Control activates randomly, especially when using wireless headphones

Voice control is going off randomly on my iPhone 12, including calling people (dial or FT) without me intending to. When the voice control comes on, there is no way to swipe to close it. I have confirmed I have voice control turned off in my settings, and it is also not activated as an accessibility shortcut. Please help me solve this!
