I have a yearly individual apple music subscription that renews in July. If I upgrade to a family plan in December and then cancel the family plan after a month, do I revert back to my individual yearly plan?

No. When you upgrade to the Family Plan, you will be credited for the unused individual plan. If you then cancel the Family Plan, you will lose any playlists you created and also any songs downloaded. The correct way to change plans is by following this support article:
