Camera Issues - auto focus not working & image stays blurry

Suddenly, my back camera seems to have a glitch. When I open it, the thing I am looking to take a video of is completely in focus, once I start recording it changes. You can tell the phone is trying to autofocus. It's moving in and out and blurring it more with each try on its own.

Is there something from the latest update that has caused this for other users? I can't seem to fix it & it is awfully annoying.

i have a 12 mini that is fairly new so it shouldn't be having this problem.
