What size and year model iMac?

Shutdown, unplug, check and clean (vacuum) the air intake on the bottom, plus the intake and vent on the back.
If it is older and the intakes/vents are real dusty, then might be best to schedule a service visit for full cleaning.
If it's an older iMac's with spinning HDD, then chattering, fluttering or clicking might suggest eminent HDD failure.
If that is the case, then it would wise to backup and schedule a service visit for evaluation and replacement.
2017 iMac serial # C0***1G5. There are two vents in the middle of the back, somewhat blocked by the stand. I have used a powerful air spray several times. It seems to have helped some, but the noise continues about 50% of the time. It flutters for a few seconds, and then stops for a few seconds. It seems to stop for longer periods when I am not using it. Thanks for any advice.
