iTunes Remote app won’t stay connected to iTunes Library

For the longest time now, the iTunes Remote app will not stay connected to my iTunes on PC. Each time I have to go into iTunes and turn off Home Sharing, then turn it back on again. After the ‘Home Sharing is Enabled’ dialog pops up, the remote works again, but usually only for a couple of hours.

I wish I could connect Remote using the 4-digit code method, but I see no Remote Icon anywhere on iTunes. In iTunes Preferences -> Devices I see no remotes paired, whether they are functioning at that time or not.

The issue occurs on all three of my devices (two iPhones + iPad). Have tried uninstalling and reinstalling the Remote app many times (v4.5.1). I’ve also reinstalled iTunes a few times (for other reasons).

iTunes on my PC is v12.13.13 (the one on Windows App Store). I’m using Windows 10.
