is it really from apple, or my phone has been hacked? This occurs only recently and keeps popping up even after I ignored it a few times. If it is from apple, why it's not included in the regular updated IOS version?

Understood. I was wondering why Apple would allow things like this to happen. Is IOS getting closer Window OS? I wish I could capture a screen and post it here, just to avoid potential confusion, but I'm not sure when it would come back again.

As was already stated, it isn’t from Apple.
Apple can’t control what websites you go to, and you wouldn’t want them to. And it can’t control what those sites do. Go to Settings/Safari and tap Clear Website History and Data.
