Safe Boot Kernel Panics Mac Air 2019 Ventura 13.2.1

My Mac Air is consistently kernel panicking even in safe mode. The error logs (attached) attribute the error to bridge_os.

Crashes occur much more frequently when I run more than one program. E.g. I can run PowerPoint OR Safari, but not both - especially if I open more than 4 or so Safari tabs. Related to this, on a reboot after a panic, it’s a race against time to force quit PowerPoint and Safari before they overload the machine, otherwise it panics again. Due to the panics, I’ve been unable to upgrade to Sonoma. I’ve tried in Safe Mode and Single User mode; single user seems to have the most success in staying stable and allowing me to work at least a little bit. I’ve re-downloaded the OS over the internet using all available options. Disk utility returns nothing amiss. Crashes occur with or without any other devices attached.

I have the whole error log as a pdf (892pages) but don’t know how to attach it (too much text to copy paste as it crashes the pdf viewer on iPhone)

At the bottom of the "Panic log shortened" is mentioned "ANS2 Recoverable Panic -- assert failed.....MD_UNCORR_ERR F2H" seems to be related to the Apple SSD in some way....ANS2 == Apple NVMe SSD v2. This is only the second time I've seen this type of panic associated with an Intel Mac....previously I had only seen it with Apple Silicon Macs. There is not much information about it, but iFixIt has an article about interpreting Kernel Panic on iPhones which mentions it is most likely a hardware failure. I think combined with the "ProxiedDevice-Bridge" panics, it is most likely a hardware failure.

Piece of advise if I may.


If this issue is truly a Hardware Issue.


I sense the 2 previous Contributors are both correct


Whether you upgrade to macOS 14 Sonoma or stay on Ventura is not going to Fix the Hardware issue


Have the computer evaluated by the Professionals who have the Special Hardware Software and Expertise to diagnose the issue and offer possible solutions


Make an Appointment at the Apple Genius Bar


Call 1 800 MyApple and have them evaluate the computer

The first paragraph is pretty much useless, but then most of it is. The only value is the kernel extensions in backtrace and the last loaded kext. What's in the backtrace may only give you an idea of what is broken. Last loaded kext is only useful if it is a third-party kernel extension.

Since it panics in Safe Mode (third-party kernel extensions disabled), it is likely some hardware fault.

If you have any devices connected besides Apple keyboard and mouse/trackpad, disconnect and test.

Apple Diagnostics will probably not report any problems for internal hardware faults that cause a kernel panic. You have to take it to an Apple Store or authorized repair center for diagnosis.

Ok thanks for the help - I figured out how to copy paste the log, but unfortunately it exceeds the character limit, so I've taken parts instead which might be of interest; I found another thread from @HWTech on a very similar issue for Sonoma (Sonoma 14.1 - Kernel Panic - Bridge OS 8.… - Apple Community). Mine logs appear slightly different but otherwise mostly the same (chip/RAM/hardware related). I'll try giving those suggested solutions a try.

Additionally, as I'm writing this, for the first time the Mac has given a Crash report in the console for "familycircled", "Code 11 Segmentation fault: 11" (crash report attached); I've not done anything differently and don't know if this is related but it did appear timestamped after a crash.

I’ll try my best to update to Sonoma and try a factory reset from a backup in the unlikely event that changes anything. In the mean time, is there a way to prevent the Mac from reopening applications after a crash (e.g. PowerPoint)? I’ve turned off all possible startup programs (except security scanner), but PowerPoint/Safari etc. still reopen after a crash and cause the Mac to crash again unless I can quickly force quit PowerPoint within 10secs of reboot.
