"Do Not Disturb" in my iPhone keeps turning on automatically

I turn off Focus / Do Not Disturb and it keeps turning itself back on. I have checked and rechecked all settings at least five times each. Ringer volume is turned up, etc. DND sets itself for 9-5 every day. I even tried setting Focus time to 1:00-1:02 AM and phone is still muted during the day. 😡
