MacBook Insurance Charge

Recently my dad purchased a MacBook Air with the three year insurance. It came out to be about $1,800. Today he was hit with an extra $200 charge from Apple saying they charged him for the insurance on the MacBook. So now he was charged a total of about $2,000. Why is there an extra insurance fee if the previous $1,800 already included the insurance charge?
