iOS Health App: Duplicate Categories (NOT Entries), Cannot Edit "Favorites"

I had to reset my iPhone to factory settings and restore from a local backup yesterday, and now when I launch the Health app it displays two of every Activity, Body Measurement, Mobility, and all other categories. On top of this, I am unable to add/remove categories from my "Favorites" - using the "Edit" feature does nothing (I can click on the stars to add/remove categories, but removed categories remain and added categories do not show up). This is crazy annoying.
