I’ve seen the below pop-up several times, but I have no idea what it is. There’s no app on my phone called this and the internet isn’t very helpful. Does anyone know what it is and how to deal with it? It’s essentially asking me to turn off the private relay so it can monitor my activity, but I’m not exactly comfortable with that.

It seems to be related to the wifi network used by Walmart and other stores and/or T-Mobile's passport. That message popped up on my phone while I was in Walmart. It's not an app on your phone, it is your iphone trying to use this wifi network.
Don't turn off private relay - double check that you have private relay turned ON for your iCloud.
