Disable and remove focus app
How do I remove a focus permanently off my iPhone 15+

There is no available Focus app and therefore there is no app to remove/delete. What you can do is remove a particular Focus from the Settings app. Open Settings ➡️ Focus ➡️ Tap on the Focus of your choice ➡️ Delete Focus (Scroll all the way down).
