Speak Screen not working from Back Tap

Same here, except Speak Screen doesn't work no matter what I set the back tap to. I use two backtaps for app switcher and three for Speak Screen, but the latter stopped working a couple of weeks ago. It's not the backtapping because when I swap the two uses, triple-tapping does app switching and double-tapping doesn't do Speak Screen. And the two-finger drag-down from the top speaks the screen throughout all this, so it's not specifically that function, either.

I have the exact same issue and as you said this happened as a result of the last update. I know this because I use this feature daily!

The Last update has significantly reduced my productivity and altered my workflow.

Ummmm, yeah. I know that. I was not asking you to fix anything. I assume that Apple engineers look at these forums once they get enough attention from the user community.
