What is going on with Focus? Why is it so hard -- nay, in my case, impossible -- to turn off the Do Not Disturb activation?
I have found a quick and seemingly foolproof workaround though -- set the schedule from 12:00 AM - 12:01 AM.

Are you noticing that the Focus gets enabled in certain locations? Your device will learn when you typically enable Do Not Disturb and then enable it automatically for you going forward. Geo-awareness.
Under each Focus group, check that there's no location awareness enabled. This is separate from the schedule settings. It enables Focus when you enter a known location.
Do you have "Share Across Devices" enabled? With that on, if you go into DND mode on a Mac, it may activate it on your phone automatically.

What happens if you just remove the schedule all together? The Do Not Disturb Focus does not just come ON on its own. If it is, then there has to be an underlying condition that is causing this. Do you use Automations within the Shortcuts app?
