How do you unlock my iPhone after forgetting my Apple ID and password?

I have locked my iPhone 5 activation lock
and I forgot my Apple ID and passcode
please help

Unfortunately, the phone is going to be useless until you can enter the correct Apple ID and Password.
Last resort would be to submit an Unlock Request to Apple. This would include documentation of the original bill of sale stating that you are the owner of the phone. This process typically takes 4-6 weeks.
