Nope. As rare and as targeted as malware is, what malware is around tries very hard to not be obvious.

But here, the built-in Apple anti-malware scanner doesn’t seem to think there’s any malware present on your iPhone.

There are many add-on security to choose from of course, and some of the better known add-on tools for Apple platforms are either solving problems that haven’t existed for a decade or so but are perfectly positioned to collect your personally-identified metadata for analysis, and some have collected and re-sold (and gotten fined) that personally-identified metadata, including online purchase info. And as mentioned above, add-on anti-malware can’t scan your device contents (the built-in anti-malware blocks that), though it can scan your network traffic.

Put differently, some of the add-on security tools are difficult to distinguish from actual malware.

No. But, if you are so concerned, back this up and then reset it to factory settings, as if you were to sell it: What to do Before you Sell, Give Away, or Trade in your iPhone or iPad - Apple Support. The SIM card will not be affected, and you phone number will reside.

No; Apple's sandboxing prevents malware from affecting it, and any scanners from checking files other than their own documents.
