How come I cannot use ecg on my Apple Watch

What model Apple Watch do you own? If Apple Watch SE, it does not have the hardware necessary for ECG.

If your watch is one which can take ECG, you must live in a country, where the ministry of health has approved ECG. This link from Apple lists the locations where ECG has that approval --> Take an ECG with the ECG app on Apple Watch - Apple Support (click on the link "where the ECG app is available). And finally, if you are under a family share plan, you can't use ECG either.
