Chances are you won't be able to do anything with it. Read this and note the paragraph at the end:

To open .app files on iPhone, follow these steps:

  1. Make sure your iPhone is running iOS 11 or later, as the Files app is available from this version onwards.
  2. Open the Files app on your iPhone. If you don’t see the blue Files app icon on your Home Screen, swipe down and search for the Files app.
  3. In the Files app, search or browse for the .app file that you want to open.
  4. If the .app file is stored locally on your device, you can find it under Locations, in On My [Device]. If the file is stored in iCloud Drive, you can find it under Locations, in iCloud Drive.
  5. Tap on the .app file to open it. If you have an app installed on your iPhone that can open the .app file, it should open automatically. If not, you may need to download an app that supports the file format or copy the file to your computer to access it.

Remember, .app files are typically application files, and they may not be directly accessible or openable on an iPhone like other document files. The steps above are for accessing and managing files in the Files app, which may include .app files. However, opening the .app file itself might require a compatible app or transferring the file to a computer for access.
