iPadOS 17.4.1. The flag icon on all mail items in all mailboxes is solid red. If I click on it, it is outlined in red.

Expose the Mail sidebar by tapping the Sidebar button at top-left of the screen:
From the Mail message list, identify the flagged mail message. With your finger, slide to the left to expose the More, Flag and Delete buttons.
The More button on the iPad opens the All Inboxes side bar, I don't get the side bar you're showing with the additional options in your 5:46a response.
What I am noticing after comparing the iPad with my MacBook is that the default flag color on the iPad is red while it is green on the MacBook. These emails are not flagged so I am actually looking at is the "Flag This Message" icon. I guess because it is red I'm thinking the email is flagged, while the flag appears below the date in the email header if I flag it. I'm dealing with a fractured knee cap and the meds are probably messing with my mind.
If you are having difficulty finding the various controls, try using your iPad in Landscape Orientation instead of Portrait.
Depending upon iPad model and screen size, while the controls are the same, the screen presentation may be slightly different.
I can’t recreate the sidebar you’re see in portrait or landscape. Are you running iPadOS 17.4.1? I’m not going to worry about it. I don’t know why my flag icon defaults to red unlike my other devices but it really doesn’t matter.

Instead of tapping the Flag button, instead select the More button. From this menu, you have more options for setting and clearing the Flag and its associated colour:
I don’t see how to bring up the window you show in your reply. This is what my screen looks like with the email open:
