sign in with passkey always fails the first attempt

when I try to sign into a website (like this one) using a passkey QR code i have to scan with my phone, the pop up passkey window on my phone always gives me the error message "the operation could not be completed. Please try again". a second attempt always works.

Passkey viewable under Passwords but not populating at specific site Created a passkey for a specific site. I can see the passkey in the Passwords Yet, when I scan the QR on the site to login, it tells me "There are no matching passkeys saved in your iCloud Keychain for "" I tried airdropping the passkey to my iPad, again I can see the password in Password, but I get the same message. I've triple check the site domain, and it appears exactly the same as in passkey entry, and the message. I added it again, just incase there was a hidden space or unprintable character. Login buttons won’t respond On iPhone 11 Login buttons and some submit buttons don’t respond on some websites/apps. Have tried using Safari and Chrome. Any suggestions?

I want to add my comment that this exact thing happens to me. Every time I use passkey, it fails the first time. Scanning again goes through every time.
