How do I know if my files are backed up to iCloud?
Most of my files are in 'Documents' and have not been moved to iCloud. Are my documents backed up by Apple?

No, if you are not using Desktop and Documents iCloud, your files are not backed up (synchronized, really) in iCloud.
To backup your Mac, use Time Machine: Use Time Machine to back up - Apple Support.
