Why is imessage showing I added/removed someone to a group text when I didn’t?

I was recently added to a group chat. After being added I received a message within the text thread that I had been added by the creator of the thread, I gave it a thumbs up & put the phone down. A few hours later someone sent a message in the thread & when I went to read the message, I noticed it said "you(me) added "name" and you(me) removed "name" But I had not added or removed anyone. The phone was sitting on the charger the entire time. Any idea why this happened?
