I run a continuous glucose monitor and an Insulin pump apps that are required to be on 24/ 7. My phone does not automatically back up. I have iCloud back up inabled and can perform a manual backup. Can the phone be configured to auto backup while on 24/ 7?

Yeah it can, in settings>your name>icloud backup set it to backup everyday when the iPhone is connected to a wifi network and on the charger
Backup is not automatically running on my iPhone
