How can an unknown third part enter an iPhone text chain?

Yesterday we had an unknown third-party enter an iPhone text conversation. The message was using a member of the text chains identity but we know it wasn't that person. That iPhone was in another room of the house at the time .... and we know it wasn't someone else accessing that device. Anyone else ever experiencing something like this? Can someone spoof another persons device through some type of hack?

Thanks for the clarification.

In that case, it appears that your relative’s AppleID is quite possibly compromised; allowing someone else to login to his/her AppleID and send iMessages in his/her name.
