Apple Pay uses NFC which is used to complete transactions with merchants when you make a purchase/transaction in person. Your iPhone only shares encrypted data with the merchant. The merchant then sends the encrypted data over the internet to their gateway (credit card processor) for approval by your bank.

If your merchant uses a VPN to as a security measure to prevent hacking, it’s conceivable that you might see multiple locations. This would apply also to Apple Pay transactions done through a website.

Where are you seeing this information? If I have an idea of the source I can probably answer your question.
