FindMy. “Always” option unavailable

Why doesn’t find my iPhone have an “Always” option for location access? How can I find my phone if it is off, because it set at “while using the app only”? Is this something that will be addressed in the future?

Your iPhone will always be findable if you've turned on Find My in Settings > [your name]. The setting you mention is just when you are using the Find My application on the iPhone itself.

You'll be able to find your iPhone at all times by logging on to iCloud.com on any other device and using the online Find My.
