Shortcuts app - Does it send notification if you DID NOT set up any shortcuts or automations?

No, it should not. Besides, only automations should display a notification when run (if you choose to receive a notification when setting one up.) Regular shortcuts do not send any notifications.

If you have other Apple devices using Screen Time, it is possible that they share their data with your iPhone. In Settings > Screen Time, the Share Across Devices setting would allow that.

Since you have no shortcuts or automations, try to delete the Shortcuts app (not just from the Home Screen.) You'll be able to download it again from the App Store if you need to use it in the future.

You could even try to redownload it right away as deleting the Shortcuts app would remove all the settings it contains and thus perhaps remove the notifications you see in Screen Time.

Could you post a screenshot of those notifications in Screen Time? I’m try to understand what you observe.
