Sorry Lawrence, but there's no way to do that form there. The website just lists the devices already logged in to your Apple ID.
You can't just add a random device like that.
To add a device to an Apple ID, you have to log in to the device with the Apple ID.
If the iPhone and the Mac are using separate Apple IDs, then they simply will not be seen on each other's Find My App.

For an Apple Mobile device:
1. Open the Settings app.
2. Tap Sign in to your [device].
3. Enter your Apple ID and password.
4. If prompted, enter the six-digit verification code sent to your trusted device or phone number and complete sign in.
Sign in to the phone using the same Apple ID as you use for your MacBook and it will appear in your list of devices.
