How can I fix sound coming only from the left speaker of my iPhone?

Independently of any settings, the sound, music, phone call, anything comes out on the left side only.

Yes, setting is stereo and balance is accurate, that is, if I move the balance all the way to the right, nothing comes out.

Defective phone?

AFAIK, the iPhone XR has two speakers. One is located at the bottom edge of the device, near the Lightning port, and the other is integrated into the earpiece at the top of the phone. This setup allows for stereo sound when the device is in landscape mode.

However, since you already checked all of the phone's software settings, if you are not getting any sound out of the "right" side, this could indicate (as you suspected) a hardware issue with your phone.

Have you tried any of these resets for your phone yet?
