Phone fell out of my pocket on the street.
Screen is smashed, but it will turn on.
One very big black splotch in the middle.
Above the splotch are some stripes of bright white with a few other colors 2.
Is only the screen affected?
Is it something more serious and fixable?

It's probably fixable, yes, though what that will involve and how much it will cost is something none of us here can say. Take it to an Apple Store or a service center for diagnosis and a price quote. With an iPhone 12, it may well not be worth the cost given its age.
