Where does Apple says that? You can select the "Connect to Any carrier later" option, and use any carrier you want when you buy an iPhone.

In general yes. The esim can be transferred to the new iPhone.

If so, then, yes, to get an iPhone under either option for a monthly fee, you have to have an active phone line with one of the carriers there.
It does not have to be your own phone line, and you technically only need it to go through the purchase process.
Once you get the iPhone, you can set it up with any carrier, as it won't be locked to the selected carrier when using Apple Monthly Installments or the iPhone Upgrade Plan.
Notes:
If you buy an iPhone with an AT&T Installment Plan (different thing to selecting AT&T under the Apple Card Monthly Installments or the iPhone Upgrade Program), your iPhone will be locked to AT&T and will only work on the AT&T network for the term of your Installment Plan agreement. If you buy an iPhone through Boost Mobile Financing (different thing to selecting Boost Mobile under the Apple Card Monthly Installments or the iPhone Upgrade Program), it will be locked to Boost Mobile for the first 60 days after activation.
