storage system verify or repair failed -69716

I am trying to resolve an issue and may have erased a volume instead of a USB. I have done a recovery and I am able to install Mojave and the first update after the install. The error that happens is when trying to update to the next OS update, "storage system verify or repair failed -69716". Cannot get beyond this. I have read many forums and haven't quite found my situation. Any ideas?

MacBook Air 13″, macOS 10.14

69716 System verify Storage Repair failed…

on an iMac (late 2012) with a 1TB Fusion Drive.

anyway, long story short... needed to recreate the fusion drive from the recovery terminal (after Backup, of course). After recreation, installation worked fine. 

This is definitely a Fusion hardware issue. I think that the drive is bad and needs to be replaced.

Finally resolved the issue. Thanks to a suggestion on https://macreports.com/storage-system-verify-or-repair-failed-how-to-fix/ that the MacOS upgrade be done incrementally. I upgraded from Mojave to Catalina, Catalina to Big Sur, and finally Big Sur to Monterey.
