Am no expert in this area, yet your question deserves some response.
As Apple users, like yourself, in the Community, we do not know what Apple intends.
Do your clients use iOS 17 or iOS 18 ?
With Assistive Access on iPhone, you can add apps that are specially designed for Assistive Access, like Calls, Camera, Magnifier, Messages, Music, and Photos.
You can also add other iOS apps in their original form.
You can add apps when you set up Assistive Access, or anytime after.

Re: I can’t have my low vision clients use Assistive Access until VoiceMail is supported.
Seems some features will just work differently in Assistive Access, purposely simple, basic.
