What is turning-off Alerts on my iPhone 16 Pro?

I am finding since upgrading to this 16 Pro that Alerts are being turned-off for reasons I can't discern. Just this morning again I noticed I didn't get some usual Notifications, and sure enought in Control Panel I found a red Alert bell (the icon with a line thru it). Turned it back On and it worked a few times until it DIDN'T again later this morning.


I noticed it early-on when I took delivery in late September, and it troubled me enough that I even re-programmed the former Silent switch, now the Action button I think, to do Nothing since if comes default I believe as mute/unmute. Nope, that wasn't it i.e. I wasn't accidentally pressing that.


I do also have a new Watch Ultra 2 that is set to "follow" the iPhone in re: Focus.


What could be the reason my Alerts are getting muted?

OK well no replies and I imagine none are forthcoming. I suspect the problem(s) may be related to my "stable" having an iPad Mini 4 running iOS15 and that it had been set itself to "Share [Focus] Across Devices". I turned that off and have not seen the problem since. Maybe a bug given 18 and 15 don't have identical features.
