Since the photo is so pixelated, I guess the app thought it was inappropriate. You can turn this off --> Receive warnings about sensitive images and videos on iPhone, iPad, and Mac - Apple Support
You didn't answer Jeff's question. What app flagged it? Logic would suggest you contact the app directly if they found the picture objectionable. Your iPhone didn't flag it.
