Because you have the Always On feature enabled in Settings/Display & Brightness.

I am using 16 Pro , i was facing the same issue till today , but I have changed the setting as below and now it is working.
Kept Dark mode on , Automatic Option is off ,True Tone is off ,night shift is off , Auto lock -30 sec, Raise to awake -on ,
Always display is off.
i have tested is multiple times and it is working fine !
If you are looking at the screen it will not lock. Might that be what you are experiencing? Put it down so the TrueDepth camera cannot possibly see your face and see if it locks.
it was the always on feature. I disabled it.
Try rebooting your iPhone first quickly press the upper volume button and second the lower volume button (don’t press the both buttons on the same time) and press and hold power button until it shows apple logo on the screen. This will fix

Did you notice you just responded to a 6 month old post? And that the problem had been resolved months ago?

I have the 16 Pro, I have been looking into the same issue, even though the “always on display” is OFF. This is when I put my phone down.
What I have discovered, though is that there are two more toggles above the always off display that only show up when you toggle it back ON .
So once toggled back on, I toggled off the wallpaper and the notifications. Because I believed it had something to do with it.
then I toggled the Always On display OFF again. Problem is fixed , thank goodness it was driving me crazy as well. Hope it works for you all.! Let me know if this works for anybody else. 😉

There is no time setting at all for Always On Display, it is ON or OFF, which is why I assumed you checked “Auto Lock” which DOES have a 30 second setting, and did not check Always On Display.
I was in this position too. Noticed yesterday that my phone was quite warm after being supposedly asleep in my bag. Didn’t think anymore about it till this morning when I noticed the screen wasn’t sleeping as it should after 30 seconds.
After reading this discussion I went to check my settings, but rebooted first.
I remembered changing my display zoom settings to larger text the previous day, so I switched back to default. Unfortunately now I’m not sure if the reboot or reverting this setting fixed it 🤦🏻 Turning larger text back on didn’t cause the same issue to reoccur though 🤔

Notifications are processed in foreground, so the screen will come on for a few seconds. If you are looking at the screen at that time Face ID will unlock it.

I'm with the same problem(iphone 16 pro) even with the always on display when a notification appears while locked the screen keeps awake until i manually lock the phone, i've checked and the raise to wake is also turned of.
Has anyone figured this out yet? My auto lock is set to 30 seconds, raise to wake is off, always on display is off. I even turned off the require screen on for camera option and restarted the phone. This has got progressively worse over the last week to the point the screen will not turn off unless I manually lock it. The last idea I have is to completely reset the phone and I’d prefer not to do that if I don’t have to.
