Among the most common reasons are screen brightness set very high, high processor usage which causes processor to heat to near 200° F, high processor usage also uses a lot of battery amps/wattage and a high discharge of a battery generates heart. iPads have no active cooling (fans) and uses heat pipes and metal housing to dissipate heat. This is less effective than active cooling such as fans.

The time to worry is if your ipad ever becomes 'too hot to handle', meaning if the heat is so much you cannot comfortably hold it. Then that's time to stop using and call in the pros. Up to that point, as stated, some heat is normal. You can also notice some heat generated by battery charging, again, as long as it's not too hot to handle you should be good.
