Hacked iPhone 6s

It does not take any sort of hacking to control your contacts list, that is done simply with your Apple ID/Password and making the changes on iCloud.com. Change your password and that will resolve the problem of you account being compromised.

It is highly unlikely that your iPhone has been hacked. Actually mirroring your iPhone is just about impossible. It's more likely that someone managed to get hold of your Apple ID and has accessed your iCloud data. See:

Or it could be just a technical issue with your iPhone and not that anyone has compromised your accounts. Without more details as to what exactly is happening with your iPhone, it's impossible for any of us to offer suggestions.

There is no list of sessions that will show when you or anyone else logged in. Changing your password is what is needed to prevent anyone from logging in other than you. Be aware of phishing emails/messages that appear to be from Apple wanting you to login into your account. These are scams that are trying to get your account details. Never click a link from an email and enter login details whether it looks like it came from Apple or even your bank.

Logins to the iCloud Web site would not be listed in the Recents list in the Phone app.
