Under my day/date, sometimes 4 circles appear. They may or may not be filled in. What are they?

That's the Signalstrengh indicator.
But you will only get these, if your watch switched to the mobile network. As long as your watch is connected to Wi-Fi or your iPhone, these icons will not be shown.

Could be the Workout rings, showing the progress of closing them.

I did not find any hint in the Apple Watch User guide about this, the information is only mentioned in the WatchApp on your iPhone, if you want to add the Wayfinder Watch face.
It’s not the Fitness circles. I don’t have those activated on this face. It’s in the area circled in red right under the day date. They are not present currently. That’s part of the confusion. Sometimes they show up and I don’t know why.
