I have recently noticed that I have battery usage (even screen time not just background) when I’m not even on my phone.
for example last night 4 apps were used on screen whilst I was asleep. Is this a sign of a hacked iPhone?
Also I have background app refresh turned off for all apps.
Any help would be appreciated!

It is normal. You are probably getting a Notification on those other apps and that is going to be counted as Screen Time. If you shut off Notifications for those apps, I bet they would go away. Background App Refresh would make no difference there.

Some Apple apps work in the background and will activate the screen during some operations. Are you backing up Messages to iCloud?
Some emails and messages may have a priority setting that activates the screen for a brief period.
I suggest you stay up all night watching your screen and when you see the activity on your screen report it to Apple Support and here in the support communities.

Moving forward we were just playing guessing games. If the minute or two of activity becomes an issue we can dig deeper or you can head to an Apple Store or contact official Apple Support for more potential solutions.

Perfectly normal. Something is always going on all the time.....even when you are sleeping.
That’s all normal. Sleep cycle is probably a third party device monitoring your sleep. While monitoring it accesses your Apple Watch or other device and is syncing data with other health app(s).

This is strange then. I definitely did not receive any messages in that time period as I woke up to no notifications.
