I have had issues with clearing the shell history using the built-in history command. I had better luck by Quitting the Terminal app and using the Finder to delete the hidden history & session file(s) and/or folder for the shell you are using.
Quit the terminal>secondary click on the terminal icon in dock>select quit>open the terminal again and you should have a clean terminal window
Command-K (⌘K) to clear the Terminal window.
Use this Terminal command to erase the command history: history -p
