I have tried to add my new credit card information via iPhone multiple times over several days as well as Apple website. The iPhone says "You did not complete the entire form." I get the exact same response if try to use Paypal as my payment method.

Include the CVV?

Thanks. The problem was that my phone number was dropped in my personal information. Since my name and address showed up, I did not think to scroll down to see that the phone number was gone.
