The Fitness app (on the iPhone) or the Workouts app (on the Apple Watch) are self-sufficient apps that do not require subscriptions. You can exercise (yoga, walking, running, swimming...) and record your own personal workouts.

The Fitness+ subscription is a video service that offers different types of workouts conducted by an instructor. You can use the subscription in conjunction with your Apple Watch to record your activity. For more information: Apple Fitness+ - Apple.
