I have formatted the cells numerous times by doing a "Clear All" then individually applying the Format functions to the cells, columns and rows. This is my formula:

Just guessing, but is that January in the cell Text or a Date & Time?
By default when you type January Numbers guesses you mean Date & Time. That will include a year. If you have Date&Time in the test-values and condition cells then make sure the year is the same. Otherwise they won't match.
It is often simpler to makes sure test-values and condition cells are Text.
