How do you activate the Blood Oxygen app on the Apple Watch Series 9 brought from the USA to Canada?

You don't. Devices manufactured for US distribution are not capable of using the app. It makes no difference if you change the region.

You don’t. Apple Watches sold in the United States do not have Blood Oxygen.
