Stair tracking accuracy on Apple Watch and iPhone
How do I get my Apple watch and/or iPhone to accurately count stairs?

It does not count stairs. It counts flights.
A flight is roughly 10 feet of vertical climb.
You must be moving up the flight at a good clip.
Pausing while going up or going up slowly will most likely not count as a flight.
Elevators and escalators don't count either.
