I turned off any Password app related settings, but this little buggy app pops up randomly asking for my password.
To make it stop, I enter my password, but it still keeps popping up randomly. It's a very buggy and forceful app.
This is my settings;

The last 2 times this has happened to me there was a macOS update that I wasn't aware of, and the popups stop as soon as I installed them. I just had it happen again this morning and sure enough 15.5 was available.

Much like default browser in System Setting >> Desktop & Dock

When you click on that box.
You can Change default from Safari to Firefox
Added, your original image is Not what Appears in the Password application
It looks like a presentation from Within your Third Party Password Application
Look there and see if it can be changed
Had the same problem. I found out it was my local qnap login page that refreshed every so often within a safari tab.
You can try using this command running in terminal and when a popup will occur you can check the details.
log stream --predicate 'eventMessage contains "password" --info
I don't have time to troubleshoot it right now. The "password" can be also change by "keychain".

Can You change " Set Up Codes In " from the Apple Password application to another password manager ?
I have the same issue. I've seen a lot of people complaining also in the past with previous MacOS Versions. Not sure if it's a Passwords bug or more a combination with other app running that is triggering the Passwords.
Looking forward to fix it.

Using a Third Party Password Manager when there already exits a Builtin in Password Application designed by Apple
Which, by the way, works as designed on 3 Apple Computers with Sequoia 15.3 and also on a iPhone with iOS 18.3
It is not my place to tell you what applications to install to manage your passwords
That is you free will to exercise that choice
It is my place to offer alternatives that are based upon Technical Details and from personal usages, which seem to work flawlessly
I am seeing less of the pop-up since the last update, but something is still trying to steal the focus randomly, exactly like you said, and it interrupts me. Once, it caused me to mistype an environment file, and it took me hours to debug. This is supposed to save time. What a joke.
I scanned for viruses, but there is not much to do when OSX behaves like a virus. Looks like they are forcing this app on users, it saves every credential without consent. I rarely use Safari.
i have the same issues, since months and its sooooooooo annoying.
Sometimes it pop ups every 5 min!!!!!!!!! and i dont see it as i have several monitors and just go on typing/coding.
then i realize have to select the screen press esc go back start again. 5 mins later the same thing!!!!

i already spent 3h on reddit, here and everywhere. no fix!!!!! i disabled everything password realted. i even dont use the apple plassword manager as i only use the chrome google one.

This is the annoying warning that pops up randomly. I nuked Password related processes on the OS but this little wanker manages to pop-up still.

I did try that, but I don't have any other option there. I have another password manager installed which doesn't show up there.
As I said, I don't have anything there.

What are you saying? Am I suppose to use every Bughouse Apple release like a mindless sheep?
It doesn't work for many people, apparently, and people have to use 3rd party for work.
