I call friends, and family and there is silence until I hear them say "hello." I can hear them once they answer, but their phone never rings. It has been happening for about a week. I can make the call and I can get their voicemail or speak to them once the call is made, but I never hear their phones ringing. Any ideas?

I did testing with 3 different devices (2 iPhones and 1 Android) on 2 different carriers (ATT and Verizon). The Verizon iPhone on 18.3.1 did not provide a "ring" on outgoing calls. The ATT iPhone on 18.3.1 did provide a "ring" and the Android phone on Verizon also provided a "ring". The only conclusion I could draw from this was that a Verizon iPhone on 18.3.1 would have issues.
For now the system has been resolved, I went to SETTTINGS>GENERAL>TRANSFER OR RESET IPHONE>RESET>RESET NETWORK SETTINGS. This will delete the carrier connection info as well as all wifi networks from the device. After this, I know hear ringing when placing calls.

That is called a ring back tone. Contact your carrier for support as they are the ones in charge of it.
Check with your cellular carrier.
I am having the same issue ever since updating to iOS 18.3. Definitely doesn’t seem like a carrier issue and some sort of issue with the update. I make calls two people, and it is total silence until someone picks up for the voicemail answers.
