Green camera indicator suddenly turned on for no apparent reason on my iPhone

I posted about an incident last week where I was using my phone and the green camera dot suddenly appeared. The app that was using the camera was the camera app, and it was only using the camera, not the audio. I know for certain I hadn't clicked on the camera app at all, or swiped on the Lock Screen.

Nothing else odd has happened for about the last week, until a few minutes ago when the green camera dot again appeared for no apparent reason.

What is odd about this to me is that neither of these camera "uses" showed up in screen time, however they did show up in the App Privacy Report. The APR said the camera app was the app using the camera. I am a bit behind on my updates, and am going to update to the latest iOS version tomorrow. But how concerned should I be now that this has happened twice? I don't do anything risky on my phone in terms of sites or downloads so this is all worrying to me.

Summary: Twice in a week, the green camera dot has shown up inexplicably. The app being used is "camera" and there is no sign of that in my screen time. However, it is present in my App Privacy Report.
