Can I apply for the Apple Upgrade Program without knowing my mom's T-Mobile account information?

Hi there i have a question. So I wanna do a apple upgrade program but i do have a question. If i dont have a loan and if i dont know my moms account because im under as my moms account such as T-Mobile do i have to know my Moms Tmobile account? its also asking me for a income on the loan. what if i put $0? i really need help please and thank you :)
